At ADHD Specialist, our philosophy is rooted in personalised, long-term care. We do not view ADHD treatment as a simple transaction of "diagnosis and prescription." It is a dynamic process that requires fine-tuning as your life, physiology, and demands change.

Many patients experience a "honeymoon period" with medication, followed by a plateau. Regular follow-ups are the clinical safeguard that ensures your treatment remains effective, safe, and tailored to your evolving needs.

Your Schedule is in Your Care Plan

You do not need to guess when your next review is due.

Please refer to your most recent clinical letter. At the end of every comprehensive report, Dr Ivantu includes a specific section titled "Care Plan." In this section, you will find the agreed follow-up consultation date and time (or the specific month due).

Action Required: It is your responsibility to ensure this appointment takes place as planned.

  • Check your last letter now.
  • Mark the date in your calendar.
  • Contact us to confirm the slot if you have not received a reminder.

Note: Your GP also receives a copy of this letter. They have scheduled your prescription authorisation to expire on that specific date. If you miss the appointment, the GP’s system will automatically flag your prescription as "unsafe to issue."

The Rules of Engagement

To maintain your prescription and clinical safety, adherence to the following review schedule is mandatory under NICE guidelines:

  • Shared Care Patients (NHS Prescriptions): Minimum frequency: Every 12 months. Your NHS GP agrees to issue your medication only on the condition that a specialist reviews you annually. If this review is missed, the "Shared Care" agreement is breached, and your GP is instructed to stop prescribing immediately.
  • Private Prescription Patients: Minimum frequency: Every 6 months. As Dr Ivantu is the sole prescriber, he carries full legal responsibility for your cardiovascular and psychiatric safety. We cannot issue controlled drugs without this semi-annual check.

Prevention is Easier than Treatment

In psychiatry, it is significantly easier to prevent a relapse than to treat an acute episode.

A follow-up is not just a "tick-box" exercise. It is a preventative medical examination designed to catch subtle issues before they become crises:

  • Cardiovascular Integrity: Stimulants are powerful vasoconstrictors. We monitor for silent hypertension or heart rate changes to prevent long-term cardiac strain.
  • Tolerance & Efficacy: We adjust your titration to ensure you stay in the "therapeutic window"—preventing the medication from becoming ineffective or causing "zombie-like" blunting.
  • Holistic Review: We review lifestyle factors, sleep hygiene, and executive function strategies to ensure the medication is supporting your life goals, not just masking symptoms.

Why Regular Follow-ups Are Essential for Shared Care

Your Responsibilities

We advise all patients to log in to their Patient Portal and review the document titled "Shared Care Agreement."

By accepting NHS funding for your medication, you legally agreed to the Patient Responsibilities section, which mandates attendance at specialist reviews.

Changing Circumstances & NHS Transition

We understand that financial situations can change. If you find that you can no longer afford private follow-up care, please do not simply stop communicating (or "lose contact"). This puts your health and prescription status at immediate risk.

If you need to leave private care, you must take action:

  1. Inform Us: Let us know so we can discharge you safely rather than listing you as "non-compliant."
  2. Contact Your GP: You must immediately ask your GP to refer you to your local NHS ADHD Service.

Important Warning on Care Gaps: Please be aware that most local NHS ADHD services have significant waiting lists. If you discharge yourself from private care before the NHS service has accepted you, your GP may be forced to stop your prescription during the interim period. This creates a gap in treatment that can last months or years, depending on local resources.

Making Your Decision: Many patients find that maintaining their private follow-up (once every 6–12 months) is a necessary investment to bridge this gap and guarantee uninterrupted treatment. However, we respect that this is a personal decision.

Our priority is your safety. Whether you stay or transfer, please ensure you have a plan in place to avoid sudden withdrawal of care.
